Age-Related Macular Degeneration: Symptoms, Types, Risks, and Treatment

It can start gradually and then all at once. The emergence of black spots in your field of vision can be a sign of retinal issues. Worse, it could also mean that there’s damage to your macula. That could mean age-related macular degeneration.

The macula is the part of the retina responsible for seeing what’s in front of us, as well as color. When macular degeneration damages this part of the eye, it renders the patient’s central vision practically unusable.

“Distorted or wavy vision and a decrease in the intensity of brightness in colors are possible signs of macular degeneration,” Asian Eye’s resident retina specialist Dr. Patricia G. Quilendrino says. “There’s not necessarily a cure for this. As the name suggests, it’s brought about by age.”

What is AMD?

Age-related macular degeneration, better known as AMD, is an eye disease that develops when tissue in the macula begins to deteriorate as we age. It often starts out with no symptoms until it progresses quickly. AMD also accounts for roughly 9% of blindness diagnoses globally. 

Interestingly, AMD and diabetic retinopathy actually possess identical features in terms of their symptoms and characteristics. Studies have suggested that those with diabetic retinopathy have a higher risk of developing macular degeneration. In fact, research has even found a link between diabetic medication and increased macular degeneration risk.

Possible Signs of AMD

This lack of central vision isn’t going to cause total blindness, but it will make it difficult for those with age-related macular degeneration to recognize faces, shapes, and other closer details. AMD ends up affecting the central vision but not the peripheral (or side) vision. 

Most patients with AMD will notice the following:

  • Blurry vision
  • Straight lines may appear wavy
  • Decrease in the intensity of brightness in colors
  • The appearance of a dark, empty area or blind spot in the center of vision
  • Loss of central vision

Two Types of AMD and How Specialists Treat Them

Dr. Quilendrino notes that we must distinguish between the two forms of macular degeneration: the dry and wet forms. The dry form affects 80% of cases, while the wet form is found among 15% to 20% of patients.

“The first is the dry type, which leads to a progressive thinning and deposit buildup in the retina,” she asserts. “The second is the wet type. Here, abnormal blood vessels form under the retina, which can leak and rupture–damaging the macula.”

If you have the dry type of macular degeneration, taking supplements or vitamins can delay the progression. But for the other type, it’s a different story. “The wet type is trickier. The first line of treatment for AMD is injections. We usually give injections to the eye, and in rare cases, retinal lasers,” Dr. Quilendrino says.

These injections target the regression of the abnormal blood vessels and decrease bleeding and swelling in the macula, especially if the “bulls eye” or the center of the macula is involved. The doctor also points out that studies have shown that injections can improve vision by up to 30%. Lasers, nevertheless, can also be used for cases that do not involve the center of the macula. 

Still, it’s about making better choices. “Healthier lifestyles like quitting smoking, maintaining a more nutritious diet, and avoiding UV exposure can help prevent you from getting AMD or delay its onset,” she advises. 

In the end, a consultation will determine the best course of action. “A comprehensive eye exam will help detect AMD, even in its early stages if you are not feeling anything,” she continues. “The recommendation for those 60 years and above is an eye checkup once or twice a year. And If you develop any of the signs that you might have AMD, it is recommended that you see a retina specialist.”
